COVID-19 Vaccination Declaration *

I confirm that my child / children registering in the group coaching program will be fully vaccinated from their first date returning to group coaching. I acknowledge that I am required to show proof of vaccination status to the coach or coaching business prior to play for my first on court session.

My child has a medical exemption from vaccination. I acknowledge that I am required to provide a medical certificate from a medical practitioner to the coach or coaching business stating that my child is unable to receive a vaccine dose. due to medical reasons, prior to my first on court lesson.
